Sleep and Gut Health

August 9, 2025

Sleep problems and gastrointestinal (GI) issues are significantly more common in autistic individuals than in the general population, and research increasingly suggests these systems are interconnected.

Sleep Challenges in Autism

An estimated 50-80% of autistic children experience sleep difficulties, compared to about 20-30% of typically developing children. Common problems include difficulty falling asleep, frequent night waking, early morning waking, and irregular sleep-wake cycles. These issues are often linked to differences in melatonin production, sensory sensitivities, and anxiety.

Gut Health Issues

GI problems affect an estimated 30-50% of autistic individuals. Constipation, diarrhea, abdominal pain, and gastroesophageal reflux are the most common complaints. Some of these issues may be exacerbated by restricted diets common in autism, while others reflect differences in gut microbiome composition.

The Gut-Brain-Sleep Connection

The gut microbiome communicates with the brain through the vagus nerve, immune system, and neurotransmitter production. Research suggests that gut microbiome differences in autistic individuals may influence sleep quality, anxiety levels, and behavior. Poor sleep, in turn, can worsen GI symptoms, creating a cycle.

Evidence-Based Strategies

For sleep: establish consistent bedtime routines, create a dark and quiet sleeping environment, consider melatonin supplementation under medical guidance, and address sensory needs with weighted blankets or white noise. For gut health: work with a gastroenterologist familiar with autism, address constipation proactively, and introduce dietary changes gradually. Your ABA therapy team can support behavioral aspects of sleep routines and mealtime challenges.
