Autism Developmental Milestones

June 20, 2025

Developmental milestones provide useful benchmarks for child development, but autistic children often follow a different trajectory. Understanding how autism affects milestone achievement helps parents identify concerns early without creating unnecessary anxiety.

Communication Milestones

Autistic children may develop language late, on time, or early, but the quality and use of language often differs. Some children babble on schedule but do not use words communicatively. Others develop a large vocabulary but use it in scripted or repetitive ways. Pragmatic language skills, like conversational turn-taking and using language socially, are often delayed even when vocabulary is advanced.

Social Milestones

Social milestones like social smiling, joint attention (sharing interest in objects with others), and cooperative play are frequently delayed in autism. These differences are among the earliest identifiable signs. By 12 months, most neurotypical children point to share interest; many autistic children do not develop this skill until later.

Motor Milestones

Motor milestones may be achieved on time or with slight delays. However, the quality of motor skills often differs, with many autistic children showing differences in coordination, tone, and motor planning. Fine motor skills like drawing and using scissors may be particularly challenging.

What to Do

Track your child’s development against general milestone guidelines but understand that autism creates a different developmental pattern, not simply a delayed one. Share concerns with your pediatrician promptly. The CDC’s milestone checklists and the ASQ screening tool are useful resources for monitoring development at home.
