Two Distinct Conditions

Schizophrenia and autism are separate neurodevelopmental and psychiatric conditions with different diagnostic criteria, onset patterns, and core features. Autism is typically identified in early childhood and involves differences in social communication and restricted or repetitive behaviors. Schizophrenia usually emerges in late adolescence or early adulthood and is characterized by psychosis, including hallucinations and delusions. One does not cause the other.

Shared Genetic Roots

Despite being distinct conditions, schizophrenia and autism share some genetic overlap. Research has identified common genetic variants that increase risk for both conditions. Copy number variations (deletions or duplications of DNA segments) at certain chromosomal locations have been associated with both autism and schizophrenia. This genetic overlap suggests some shared biological pathways in brain development, even though the conditions manifest very differently.

Diagnostic Challenges

Historically, autism and schizophrenia were sometimes confused. Before the 1970s, childhood autism was occasionally classified as a form of childhood schizophrenia. Today, the diagnostic criteria are clearly separated in the DSM-5. However, some features can overlap, particularly in individuals with limited speech, unusual thought patterns, or social withdrawal. Accurate diagnosis requires careful assessment by experienced clinicians who can distinguish between the two.

Co-Occurrence

While rare, it is possible for an individual to have both autism and schizophrenia. Estimates suggest that people with autism may have a slightly higher risk of developing schizophrenia compared to the general population, though the absolute risk remains low. When both conditions are present, treatment must address the unique features of each. Antipsychotic medications may help with psychotic symptoms while behavioral interventions address autism-related needs.
