Early Signs to Watch For
By 12 months: limited babbling, no gestures (pointing, waving), no response to name, limited eye contact. By 18 months: no single words, does not point to show interest, limited imitation, no pretend play. By 24 months: no two-word phrases, loss of previously acquired skills, limited interest in other children, unusual repetitive behaviors. Not all children who show these signs have autism, but they warrant evaluation.
Developmental Milestones
Compare your child’s development to typical milestones while recognizing that ranges are normal. Social milestones include smiling at people (2 months), responding to name (6-9 months), playing peek-a-boo (9 months), pointing to share interest (12 months), and engaging in pretend play (18 months). Communication milestones include babbling (6 months), first words (12 months), and combining words (18-24 months). Delays in multiple areas are more concerning than a single delayed milestone.
Screening Tools
The M-CHAT-R/F (Modified Checklist for Autism in Toddlers) is the most widely used screening tool, appropriate for 16-30 months. It takes about five minutes and identifies children who should receive further evaluation. Your pediatrician should administer this at the 18 and 24-month well-child visits. Online versions are available for parent self-screening, though professional follow-up is essential regardless of the result.
What to Do If You Are Concerned
Trust your instincts. Parents are often the first to notice developmental differences. Talk to your pediatrician about your concerns. Request a formal screening even if your doctor is not concerned; parental concern alone warrants screening. Contact your state’s early intervention program (Babies Can’t Wait in Georgia, AzEIP in Arizona) for a free evaluation if your child is under three.
