Lorna Wing and the Triad

The Triad of Impairments was developed by Dr. Lorna Wing, a British psychiatrist, in the 1970s and 1980s. Wing identified three core areas of difficulty in autism: social interaction, social communication, and social imagination (also described as flexibility of thought). This framework became the foundation for autism diagnosis for decades and directly influenced the diagnostic criteria in the ICD and DSM systems.

The Three Areas

Social interaction refers to difficulties understanding and responding to social cues, forming relationships, and navigating social situations. Social communication encompasses challenges with verbal and nonverbal communication, including tone, gesture, facial expressions, and conversational reciprocity. Social imagination involves difficulty with flexible thinking, understanding others’ perspectives, imagining outcomes, and adapting to change.

Wing’s Broader Contributions

Beyond the Triad, Lorna Wing made enormous contributions to autism understanding. She coined the term “Asperger’s Syndrome” in English, bringing Hans Asperger’s work to international attention. She proposed the concept of the autism spectrum, recognizing that autism exists on a continuum rather than as a single fixed condition. Her work directly shaped the diagnostic shift toward spectrum-based understanding.

Modern Updates

The DSM-5 (2013) condensed Wing’s triad into two domains: social communication/interaction (combining the first two) and restricted, repetitive patterns of behavior (expanding imagination to include sensory and behavioral elements). While the structure changed, Wing’s fundamental insight that autism involves these interconnected areas of difference remains central to modern understanding.
